This is Form GA-01, the intake we need before preparing a personal return. Work through it here and download the finished PDF at the end, or download the blank form and fill it in by hand.

Nothing you type on this page is sent anywhere. The PDF is built on your own device, and you send it to us yourself through the secure upload link. That matters here because this form asks for your SIN, which should never travel as an email attachment.

Every field except your name, email, and the confirmation at the end is optional. If you don't know something, leave it blank and we'll ask.

Check what you paid for — receipts make them count.

Not sure whether something counts? Check it anyway or note it in section 07 — deciding what applies is our job, not yours. It's faster to rule something out than to discover it in March.

These affect required forms and elections. Answer for the tax year above.

Did you own foreign property with a total cost over $100,000 CAD at any time? This includes crypto held on foreign platforms, and triggers Form T1135.
Did you sell a home that was your principal residence?
Did you buy your first home?
Are you a Canadian citizen, and do you authorize the CRA to share your name with Elections Canada?
Are you registered for CRA My Account?
